This job description is for a substantive consultant post in palliative medicine at Mid Yorkshire Teaching NHS Trust. This is a 10 PA post working alongside the consultant palliative medicine team at Mid Yorkshire and also our community teams crossing several divisions within Mid Yorkshire and importantly work closely with community services. Mid Yorkshire Teaching NHS Trust aims to provide a cohesive, high quality medical service to patients with specialist palliative care needs. The post holder will provide consultant cross cover for The Prince of Wales Hospice and the Mid Yorkshire Specialist Palliative Care Team for leave and absence.



This post is offered on a 10 PA basis although less than less than full time applications will be considered. The base hospital will be Pinderfields Hospital, where office accommodation is located for all members of the Mid Yorkshire Macmillan Specialist Palliative Care Team. All Mid Yorkshire consultants may be asked to work on any of the three hospital sites if needed, but this post is primarily focused on supporting community services and a Hospital at Home model of Care. Mid Yorkshire Teaching NHS Trust provide funding for 10 PAs including 1.5 PAs SPA time.



This includes on-call. For any urgent and unforeseen duties including out-of-hours on-call; schedules will take due account of the provisions of working time regulations and safe working (currently none in place). Wakefield Hospice is located across the road from the main Pinderfields site, and it is a 10-minute walk between the sites. The Prince of Wales Hospice is in Pontefract.

The main duties of the post will include:

Support for the Community Specialist Palliative Care Team with both joint and independent review of patients. Support for Gold Standards Framework meetings in primary care where appropriate. Community team MDT weekly Cancer of unknown primary MDT Support for Hospital at Home Service Support for Care Homes One in nine-week consultant on call providing telephone advice to health care professionals across Pontefract, Wakefield, Kirklees, Huddersfield and Calderdale. (Currently assessed as 0.75 PA per week plus 5% category A on call).



In the event of sickness or absence, the first on call at any of the hospice units the responsibility to find alternative cover is held by the unit and not the second on call consultant. Administrative duties, for which appropriate office space and secretarial support is provided Supporting Professional Activities in accordance with the Trust Framework, with at least a minimum of 1.5 SPAs included in the core 10PA timetable. Core SPA includes audit and governance. Additional SPA or DCC time may be commissioned in line with the skills of the successful candidate and changing requirements of the service.



JOB CONTENT This post holder will be based at Pinderfields Hospital (PGH). There will be mentorship provided from a senior member of the team within the department. The post holder will attend the weekly community MDT on a rotational basis. The post holder will work alongside 5 consultant colleagues at Mid Yorkshire Teaching NHS Trust, 1 specialty grade doctor (2 days per week), 1 specialist registrar, 15 hospital and community clinical nurse specialists in palliative medicine, an end of life care facilitator, a fast track facilitator and 4 band 4 care co-ordinators.



Expected workload oAdvice and support to community specialist palliative care team including leading the community multidisciplinary meeting and reviewing complex patients in their home with nursing colleagues, or via telephone (estimated at 6 patients per week). oSupport a Hospital at Home Model of Care alongside other specialties with an MDT format as well as appropriate clinical reviews in a patient's own home oSupport to Care Homes. Working in collaboration with Frailty Teams to improve quality oif Care and ACP. Continuing Professional Development The Trust requires all its doctors to engage with continuing professional development, guided by the relevant professional associations or Royal Colleges.



Time and funding is provided according to departmental and Trust policies, subject to approval through the clinical management structures. There is a programme of mandatory training which all relevant staff must complete as required. Research Medical staff are encouraged to develop areas of personal and/or research interest. This can be developed in liaison with the Trusts R&D department and in line with prevailing policies and research frameworks.



The Trust is developing an expanding and outward-looking research programme. Mentoring Newly appointed senior medical staff will be offered mentoring opportunities either within the department or within the wider Trust and may be specified by the appointments committee. There is an induction and development programme for all new staff. All senior doctors are involved in training our doctors, students and other professions. There are many opportunities to develop training and education roles, and many have taken on educational leadership roles within and beyond the Trust. Audit and Clinical Governance There is a regular clinical governance palliative medicine meeting. The Clinical Audit Department assists with setting and supporting an audit programme for the Service with which all colleagues are encouraged to engage.



Clinical Audit is considered within objectives as part of job planning. Representation Our Senior Medical Staff Committee provides a forum to liaise with and advise our senior management team. The BMA-accredited Local Negotiating Committee negotiates on terms-and-conditions matters. Hello, my name is Dr Hazel Pearse and I am the lead for palliative care services at Mid-Yorkshire Teaching NHS Trust and Associate Medical Director for Cancer Services.



I work closely with a team of 4 Consultants, Dr Lowe, Dr Ashwood, Dr Montgomery and Dr Schuberth. Mid Yorkshire Macmillan Specialist Palliative Care Team Mid Yorkshire Macmillan Specialist Palliative Care team provides specialist palliative care services to hospital and community patients across Wakefield District and Mid Yorkshire Hospitals NHS Trust. The team comprises 5 consultants; 1 staff grade doctor; 1 specialist registrar, the nursing team leader; 5 band 7 clinical nurse specialists; 1 band 7 End of Life Care Facilitator, 9 band 6 clinical nurse specialists; 1 fast track facilitator, 1 band 5 palliative care nurse for end of life care, 4 band 4 coordinators, 3 health care support workers and secretarial team. Palliative Medicine Outpatient clinics are run from the two local hospice sites and also The Rosewood Center.



Two of the consultants work jointly with Wakefield Hospice (Dr Schuberth and Dr Lowe) and two work jointly with the Prince of Wales Hospice (Dr Pearse and Dr Ashwood). All the consultants work together as a team to provide mutual support and as an integral part of the multidisciplinary specialist palliative care team also providing mutual support and education. Dr Lowe also provides clinical services for patients with Motor Neurone Disease (MND) as an area of special interest and leads the monthly MND multidisciplinary team meeting and clinic at Pinderfields Hospital. Dr Pearse holds the role of Associate Medical Director with a Lead for Cancer Services.



The post holder will provide community advisory services working alongside their community Macmillan nursing colleagues. They will be expected to work closely with the primary care teams. He/she will have the opportunity to work with a cancer Multidisciplinary team (MDT) currently allocated as cancer of unknown primary. The role is expected to support the Trust Hospital at Home Service as well as supporting advance care planning for community patients including care homes.



These areas of interest may be changed depending on the preferences of the successful candidate and areas of interest are discussed by the consultant team at team job planning which occurs at least annually.
