We have a great opportunity for Customer Coordinators to join our Connected Lives Gateway Service for both Older People and Adults with Disabilities. This service aims to prevent, reduce and delay need by ensuring people access a wide range of information/advice, intervention, and support at the right time and in the most uncomplicated way possible.
The Gateway Connections team is a hybrid office/home working telephone and virtual contact team all about working preventatively with adults who need information, advice, and support to stay independent in their own homes for longer. The multi-disciplinary team works with Social Workers and Occupational Therapists and other professionals to achieve the best outcomes for people of Hertfordshire.
Applying HCC's Connected Lives assessment framework, Gateway Connections work with citizens focusing on what the person wants to achieve - their outcomes. We explore personal strengths, their family and friends' support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.
We work collaboratively with our operational links in the Voluntary, Community, Faith and Social Enterprise preventative strategy, Herts Help, and social prescribing. Relationships will also be developed with Hertfordshire Partnership NHS Foundation Trust (HPFT) Single Point of Access, the Frailty single point of contact, & Health partners.
About the role
You will promote and support a strength-based approach to all interventions to maximise independence, health, and well-being by offering opportunities to prevent, delay or reduce the need for Adult Social Care. This will require you to apply your creativity working with residents to ensure their safety and continued independence. Your knowledge of local services to support people to access appropriate community resources is also crucial as will working alongside our visiting CCOs, Social Workers and Occupational therapists.
You will work successfully with partners and many other agencies for the benefit of citizens.

About you
Essential
Previous experience in a Social Care setting
Ability to work autonomously as well as part of a team
Previous experience communicating with vulnerable adults
Previous experience in customer services involving vulnerable adults
Empathic and patient when dealing with vulnerable adults
Desirable:
Knowledge of Social Care legislation such as Care Act 2014 and Mental Capacity Act 2005
