This is an opportunity to join our friendly, supportive team of dedicated professionals providing services to 14,700 pts in the Wimbledon Area, with a very recent CQC inspection outcome of Good across all domains. We are seeking an individual ideally with experience of working within primary care, with experience of data reporting and achieving targets/KPI's as well as knowledge and experience of quality assurance.
This role will be responsible for supporting the delivery of high-quality, safe and compliant patient care. The job holder will do this by developing or overseeing quality management systems, conducting audits, investigating and sharing the learning from significant and learning events.
They will ensure data is accurate and income from local and national targets is maximised. They will lead on QOF, Local Incentive Schemes, PCN and other targets. They will work with clinical and non-clinical staff to ensure these are achieved and data is submitted as required. They will also review new specifications and support the practice to implement these to achieve the best outcomes and maximise practice income.
This role will manage a small team of staff who support the practice with the administrative side of QOF etc and implementing the call/recall system.
This role will also support the practice to demonstrate high quality patient care and CQC compliance by ensuring that work we are doing is evidenced efficiently and effectively. They will utilise systems to ensure evidence is easily accessible, documented and linked to relevant policies and procedures. They will work with the CQC clinical leads to ensure we are able to maintain our current Good CQC rating. They will play an active role in any CQC inspections that take place. The role will include aspects of patient engagement.
The role will also form part of the duty management team rota, supporting the practice to run smoothly at all times.
Primary Responsibilities
The following are the core responsibilities of the Data and Quality assurance manager. You may be required to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
a. Work alongside the Nursing team and the other clinical QOF leads to ensure QOF call/recalls are completed, SMS, letter invitations, emails and calls are made in line with practice expectations and patient attendance is maximised.
b. Undertake batch messaging for patients
c. Ensure the practice stays within its fragment allocation for SMS use.
d. Monitor call, triage and appointment data, completing weekly, monthly and quarterly reports on performance.
e. Use data to identify trends and provide insights into how to use our staff and practice resources more effectively.
f. Oversight and implementation of the practices DNA policy.
g. Write and amend searches within EMIS to support achievement and quality assurance.
h. Ensure the Ardens searches are utilised to their full effectiveness to support delivery of patient care
i. Monitor exception reporting, prevalence data and patient deductions through the year to ensure we are maximising income.
j. Report on monthly/quarterly and annual data as required to the Business Manager and GP Partners
k. Plan how you and the admin team will manage the call/recalls and campaigns such as Smear/Flu vaccinations ensuring the highest uptake.
l. Support the team to improve cancer screening uptake
m. Line Manage the Care Coordinators.
n. Completion of quarterly patient death audits
o. Take the lead in working with clinicians to implement new specifications that require reporting on and ensure structures are in place to meet reporting requirements.
p. Be involved in reviewing processes to ensure data is captured.
q. Lead on the development and implementation of the annual Audit Plan for the practice, ensure 2nd cycle audits are completed, learning shared and actions implemented.
r. Undertake and document non-clinical audits to demonstrate how the practice is identifying areas of improvement and the impact change have made.
s. Use GP teamnet to demonstrate CQC compliance across all areas
t. Ensure those responsible for premises checks and other compliance checks are completing them, documentation stored and any actions completed
u. Have oversight of all policies and procedures, ensure policies are reviewed regularly and reflect the current best practice as well as what the practice procedure is.
v. Ensure we have all policies in place that we should have.
w. Keep up to date with 'Nigel's Surgery' and share information and updates with relevant staff and at relevant meetings. Implement changes to practice and policy as necessary.
x. 6 monthly completion of the CQC checklist and review with Clinical leads
y. Lead on significant and learning events, investigating the incidents and ensuring the relevant documentation has been completed and stored in line with practice protocols. Undertake the annual summary of events and regularly review to make sure the learning and changes in practice continue to be implemented.
z. Keep up to date on CQC inspection processes and requirements
aa. Work with other managers to support them in ensuring their areas of responsibility are in line with best practice and meet CQC expectations.
bb. Undertaken actions as needed following CQC inspections or CQC planning meetings.
cc. Attend monthly governance meetings and present learning from significant events and any policy changes to the team.
dd. Provide training for staff as and when required and undertaken aspects of staff induction on your lead areas.
ee. Participate in the duty manager rota, processing duty manager emails, requests and dealing with any issues that are occurring during your shift e.g. printer not working, contractors that are on site.
ff. Lead on ensuring the practice information is up to date, including the website, leaflets, newsletters and welcome messages on the phone.
gg. Send out and collate the patient survey data on a Monthly basis and share results and trends with the team. Act on feedback.
hh. Develop and keep updated Topics pages on GP Teamnet so staff can easily access centralised resources.
Minute taking as and when required at various meetings, including the managers meeting, Governance meeting, Clinical supervision meetings, Safeguarding meetings.
