Pwrpas y swydd:

Mae'r rol yn cynnwys cefnogi darparu gofal o ddydd i ddydd, gan sicrhau llety diogel o safon i unigolion ag anghenion iechyd meddwl, gan eu grymuso i fyw mor annibynnol a phosib. Byddwch yn rhan o dim, gan gynnal cynlluniau gofal personol i safon uchel, cwblhau dyletswyddau gofal gan ddefnyddio systemau TG, ac yn gallu gweithio oriau anghymdeithasol ac ar benwythnosau. Mae agwedd gadarnhaol a pharodrwydd i ymdopi ag heriau yn allweddol i'r swydd.


Cymwysterau angenrheidiol:




NVQ/QCF Lefel 2 mewn Iechyd a Gofal Cymdeithasol, neu fod yn barod i weithio tuag at hyn Sgiliau cyfathrebu da (llafar ac ysgrifenedig) Medru defnyddio cyfrifiadur (cewch hyfforddiant llawn) Gweithio'n dda ar eich liwt eich hun ac fel rhan o dim Cofrestru gyda Gofal Cymdeithasol Cymru (ar ol cwblhau cymhwyster)

Job purpose:




The role involves actively supporting the day-to-day delivery of care, ensuring the provision of good quality, safe accommodation that empowers vulnerable individuals with mental health challenges to live as independently as possible, working as part of a team to maintain individual care plans to a high standard, completing care duties and planning using IT systems, and demonstrating flexibility to work unsocial hours and weekends while maintaining a positive, proactive approach to overcoming challenges and achieving positive outcomes.




You must complete or have one of the following qualifications




Minimum NVQ/QCF Level 2 or a willingness to work towards this in Health and Social Care Registration with Social Care Wales once completed.

Effective communication skills (written & oral) are essential.


Use of computers and technology to input information and access care planning- Training and support will be provided.


The ability to work on own initiative and as part of a team.
