Job Overview


We are seeking a compassionate and dedicated Support Worker to join our team. A strong understanding of patient care, particularly for those with Alzheimer's or dementia, is essential.

Summary of Post:



Organising and participating in ACC Dementia Activity and Support Groups (DASGs) and other related dementia services

Liaising with Northern Health and Social Care Trust (NHSCT), Western Health and Social Care Trust (WHSCT), Alzheimer's Society and other statutory/voluntary organisations' staff re referrals for the DASGs

Carry out home visits to assess suitability for "Better Together" programmes and conduct carer assessments.

Ensure baseline information on service users is collected.

Assist in the collection and collation of information relevant to dementia and community support programmes.

Assist in recruitment of and support to ACC volunteers involved dementia programmes and community mental health support services.

Support carers of vulnerable adults throughout their direct involvement with ACC and beyond.

Responsibilities



Develop positive relationships with the service user's carers, family and friends to help maintain the service user's skills, social activities and relationships. Refer any concerns to the service development manager.

Organise stimulating activities within the agreed support plan, considering service user's interests and range of available resources.

Monitor the support plan, keep a diary of relevant observations and notes and report to the Manager all matters relevant to the service user's care.

Ensure up to date records are maintained and to assist in collating any relevant service records to meet required deadlines.

Encourage, develop and implement activities, by maintaining the person's skills and interests. These may include crafts such as knitting, sewing, gardening and art, games, music and/or dancing as well as social outings (subject to Covid-19 restrictions)..

Facilitate and carry out agreed structured creative communication, to assist the individual retain their sense of identity. This may include, for instance: reminiscence work; life history albums; reassurance techniques.

Provide opportunities for individuals to participate inclusively in a dementia and life skills project.

One-to-One Home Support



Visit and provide stimulating activities for clients who may, for whatever reason, be unable to partake in group activities.

Qualifications



Essential Criteria



Minimum of 5 GCSE/O Level

and either

a minimum of at least one years' experience of working with families and people living with dementia in the community, voluntary, private, or statutory sectors

or

a minimum of two years in a voluntary role working with people living with dementia in the community, voluntary, private of statutory sectors

Desirable Criteria



Ability and confidence to lead a Dementia Support group, providing stimulating fun activities for people living with dementia. The ability to engage in conversation and provide topics of conversation.

Good administrative and organisational skills and the ability to use computer software. e.g. Microsoft Word, PowerPoint, Excel and Zoom.

Knowledge



Knowledge of people and families living with dementia and issues affecting the health and welfare of older people

Skills



Ability to communicate effectively both orally and in writing for internal and external audiences.

Ability to work effectively as part of a team and as an individual.

Ability to be creative and adapt to induvial client's needs.

Special aptitudes



Applicants should possess a driving licence and have access to a car, or have access to an appropriate means of transport to carry out the requirements of the job.

Have a passion for working with vulnerable people.

Knowledge of Alzheimer's care, dementia care, is highly desirable. Certification in first aid is an advantage. Strong interpersonal skills with the ability to build rapport with clients and their families. Ability to work independently as well as part of a team. A valid driving licence is preferred for transportation duties. Flexibility to work various shifts as needed to meet client requirements.
