Purpose of the Post
To provide a personalised, single point of access support for carers of someone living with dementia. Carer support will be practical, financial and emotional with the aim of significantly improving the well-being of carers/their families to maintain and maximise their quality of life and mental wellbeing.
Main areas of responsibility
Engage with Carers of individuals living with dementia both pre and post diagnosis
Offer guidance and 1-2-1 support to carers supporting them through the caring journey, paying particular attention to loss, transitions and changing relationships
Offer Preventative/Self-care opportunities for carers, individual carer support, support at times of carer crisis and support through outreach work in hospitals
Deliver training sessions and signposting to online resources to support the carer in their understanding of dementia
Liaise with professionals to raise awareness of the project and increase referrals to Swansea Carers Centre
To organise own work and work as part of larger multi-professional team.
Maintain good records of work undertaken using Charity log for monitoring and evaluation purposes
General
To participate as a member of the staff team, attending Caring Break team meetings and other Swansea Carer Centre meetings as required.
To work at all times within the philosophy and policies of Swansea Carer Centre.
To comply with the Health & Safety at Work Act 1974 and with Swansea Carer Centre policy, paying particular attention to the reporting of dangerous situations.

Person Specification
Essential Knowledge, Skills & Experience
Understanding of older people and people with Dementia and issues effecting their quality of life
Understanding of the needs and problems experienced by family carers caring at home for a dependent relative with particular emphasis on Dementia
Knowledge of local and national dementia related support
Experience of working in collaboration with statutory organisations such as Health and Local Authority
Experience of delivering 1-2-1 support
Ability to maintain and respect the value of confidentiality
Flexible approach to working practices
Sense of humour
Experience of CRM databases and MS 365 packages
To participate and undertake any training as required
Knowledge of Health & Safety requirements
Desirable Knowledge, Skills and Experience
The ability to speak Welsh
Experience of working in similar area or field
Experience of delivering training or workshops
