What You'll Do

As our Care Coordinator, you'll be

the heartbeat of our new Chelmsford service

, ensuring smooth operations and exceptional care for our clients.
Your role will include:

Coordinating rotas and visit schedules to deliver consistent, high-quality care. Building strong relationships with clients, families, and healthcare professionals. Monitoring care quality and ensuring CQC compliance from day one. Supporting and mentoring our care team to help them thrive and grow. Sharing participation in the on-call rota (1 in 4 weeks) to keep services running seamlessly.
What You'll Bring

We're looking for someone with a

mix of compassion and organisational skills

, ready to step into a leadership role.
You'll need:

1-2 years of experience in a care setting, ideally in domiciliary/community care. At least 1 year of care coordination experience. A Level 3 Health & Social Care qualification (or willingness to achieve it). Excellent communication and problem-solving skills. Confidence using digital systems for scheduling and record keeping. A full UK driving licence and access to a reliable vehicle. A proactive, empathetic approach with the ability to stay calm under pressure.
Job Type: Full-time

Pay: 28,000.00-30,000.00 per year
