Proclaim Care has an excellent opportunity for an experienced Occupational Therapist, Nurse or Physiotherapist to join our team as a Desktop Rehabilitation Case Manager. Part of Proclaim Group, we are a national, award-winning provider of rehabilitation solutions for individuals who have been injured and/or are absent from work.


This is a home-based role suitable for a candidate based within the UK.


Candidates must be willing and able to travel on an occasional basis to our head office in Hamilton and nationally to attend training and team meetings.

This is an exciting opportunity to support service users with injuries and health conditions to optimise their recovery and reach their rehabilitation and return to work goals. Working within a commercial (personal injury and income protection) context, this desk-based role involves navigating service users through their personal rehabilitation journey by:


Conducting virtual (telephone and video call) assessments to determine service users physical, psychological and social needs on an ongoing basis Devising and coordinating goal-focussed rehabilitation programmes, which includes a combination of both private and statutory services Coordinating private treatment e.g. physiotherapy, psychology, investigations or surgery Collaborating with family members and other inter-disciplinary professionals Communicating regularly with external stakeholders e.g. solicitors, insurers and employers Writing reports for the instructing parties to provide updates, demonstrate progress and seek funding approval Facilitating return to work and/or education

Person Specification:




We are looking for a HCPC registered Occupational Therapist or Physiotherapist, or NMC registered Nurse with considerable experience in liaising with employers and supporting people with injuries and health conditions to remain in, or return to, work.


Desktop case management, DWP or occupational health experience is desirable, but not essential as full training can be provided.


Our Desktop Rehabilitation Case Managers work from home but regional travel to attend training and team meetings is required. A valid driving licence is therefore essential.
