You will be part of the Dietetics Team within the Hounslow Community Diabetes Service (HCDS) delivering the award-winning structured education programme X-PERT to adults referred with type 2 diabetes in the London Borough of Hounslow. This will include teaching in group sessions, face-to-face session and via virtual platforms.
Hounslow has a diverse population, and you will also support on delivering the structured group education programmes in the South Asian dialects (Hindi, Urdu or Punjabi) using your own language skills or use of interpreter services.
The part time Band 4 Diabetes Educator Facilitator post would be suitable for an individual with an interest in health and wellbeing/nutrition. The successful candidate will be able to demonstrate strong teaching skills, excellent interpersonal, communication and organisational skills together with an ability to use their own initiative. The post holder will work 18 hrs/week to suit service needs. This may sometimes include some evenings and weekend shifts.
Main duties of the job
As a proactive member of the team your primary duties will be to assist in the delivery of X-PERT to patients in group settings, which will require travel to venues across the borough of Hounslow. The role also supports the HCDS in the delivery of training and education to other health care professionals and at public engagement events. You will also provide administrative support for the HCDS team to meet service needs.
