Job Description

This role is for 15 hours per week to be worked on Monday 0800-1300 and Fridays 0800-1830 (with a 30 minute break). To be responsible for the management of diabetic patients, delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The Diabetic Nurse will also be responsible for a variety of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.
