The Role
We are seeking an experienced Practice Nurse with a specialist interest in diabetes to join our supportive and forward-thinking team. You'll work across our sites delivering high-quality care, leading diabetes reviews, and supporting patients with long-term conditions.
Key Responsibilities
Carry out diabetes reviews and contribute to long-term condition care planning
Monitor insulin and other diabetes medications as part of patient management
Provide diabetes education and clinical guidance to the wider primary care team
Run general nursing clinic, including venepuncture, ECGs, BP monitoring, wound care, and health screening
Collaborate with the wider clinical team to ensure continuity of care
Contribute to minor illness and general treatment clinics (if qualified)
About You
Essential:
Registered Nurse with current NMC registration
Experience managing diabetes in a clinical setting with a relevant nursing degree
Excellent clinical, communication, and IT skills (including EMIS or similar systems)
Strong clinical, communication, and IT skills
Confident working independently across multiple sites and within a multidisciplinary team
Commitment to professional development and clinical governance
Knowledge of EMIS web or other clinical systems
Desirable
Minimum of 2 years' post-registration experience in diabetes care
Experience working in primary or community care settings
independent Prescribing qualification (or working towards it)
