Job Description

Provide specialist diabetes care to patients within the primary care setting, in line with NICE guidelines and local protocols. Conduct comprehensive assessments and reviews of patients with complex diabetes, including insulin management and medication titration. Support GPs and practice nurses with treatment plans and clinical decision-making. Lead and contribute to the delivery of structured diabetes education programmes (e.g., DESMOND).

Promote self-care and lifestyle management among patients to support glycaemic control and reduce complications. Work closely with the wider multidisciplinary team including dietitians, podiatrists, and secondary care DSNs. Undertake audits, quality improvement initiatives, and contribute to QOF/PCN targets related to diabetes. Provide training and clinical supervision to practice staff in diabetes care and management.

Keep accurate, contemporaneous records on clinical systems (e.g., SystmOne) Engage in patient recall and proactive case-finding to identify those at risk of poor control or complications.
