Are you looking for a career where you can make a difference and provide excellent care to those who need it?This is an exciting opportunity to join our team in Urgent and Intermediate Care in Pembrokeshire.We are an innovative, comprehensive multidisciplinary team (MDT) of Dietitians, Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Nurses, GPs, Advanced Nurse Practitioners, Advanced Paramedic Practitioners, Advanced Clinical Practitioners, Physician Associates, Social Workers, Reablement Workers and Third Sector Agents striving to deliver the right care and support at the right time in the right place for patients.The Intermediate Care MDT work together to deliver patient care in a holistic, person-centred manner, supporting patients, to stay/return in a safe manner to their own homes in line with Welsh government 6 goals policy to enable improved care, outcomes, experience and value.The post holder will assist with nutritional screening and cascading nutritional knowledge to the MDT and public with training and education sessions. They will support the Dietitian and wider MDT to identify patients at risk of malnutrition, by conducting nutritional assessments and providing nutrition support. The primary aim being empowering patients to improve their nutritional status and prevent deconditioning by making achievable positive change.Main duties of the job

  • Support the delivery of a variety of specialist nutrition and dietetic assessment , treatments and interventions in line with agreed guidelines,protocols and care pathways.
  • Develop andmodify nutritional care plans for patients with predictable recovery / deterioration.
  • Support monitoring and evaluation of nutrition and dietetic interventions in line with prescribed care.
  • Demonstrate significant knowledge of nutrition and the impact of the patient medical condition in order to adapt any nutrition and dietetic guidance given and use initiative appropriately based on presentation.
  • Work in partnership with pharmacy, GP Practices, social care andthe MDT to ensure nutrition and dietetic therapeutic intervention is effectively facilitated.
  • Actively support improvements in standards of nutritional caretransferrable from the acute to the community setting.
  • Evaluate the effectiveness of nutrition interventions and care, including monitoring of systems and processes undertaken by teams outside of dietetics.
  • Support the dietetic team with planning , delivery and evaluation of scripted education and training sessions to a range of staff and patient groups.
  • Problem-solve to reduce the impact of nutrition related issues on patient outcomes.
