The Integrated Discharge Team (IDT)are looking to recruit a motivated and dynamic Band 5 Occupational Therapist to join our team.

IDT is a valued and integral part of UHCW flow and the Integrated Care and Community Services group which is continually evolving in order to best meet the needs of our patients.

Our service covers 7 days and extended hours (08:00-18:00 Monday-Friday).

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.



We welcome applications from Occupational Therapists currently practising, returning to practice or those that are newly qualified and have an interest in Discharge.



Working alongside an interdisciplinary, knowledgeable and supportive team the successful candidate will have the opportunity to develop complex assessment and decision-making skills in discharge.



The post holder will provide assessments, coordinate and refer patients for support along their ongoing journey of recovery and / or reablement to maintain, and restore functional abilities whilst considering the individuals' abilities, health requirements, and environment.



The post holder will demonstrate excellent communication, interpersonal and team-working skills. The post holder will be expected to manage a caseload, plan, and prioritise assessments, whilst demonstrating clear clinical reasoning and evidence-based practice, with support from their team and senior colleagues.



The post holder will actively engage to promote the role of Occupational Therapy and the Integrated Discharge Team Service.



The post holder will provide clinical support to less-experienced staff, as delegated by seniors.



Staff are also supported by the AHP Faculty who provide regular training sessions, alongside the team In-Service Training opportunities.

Work in accordance with COT & HCPC rules of professional conduct and within local care pathways, policies and standards.



Professionally and legally responsible and accountable for all aspects of his/her own professional activities and to manage clinical risk within own caseload at all times, whilst remaining responsible to Team Leader/Clinical Specialist.



Promote awareness of the role of Occupational Therapy within the IIDT.



Work as an autonomous member of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care and discharge pathways



To represent IDT at the board rounds, proactively driving discharge, questioning discharge plans, predicted discharge dates and escalating delays to managers.



Hold responsibility for own caseload assessing their health and social care needs, to ensure the correct level of support is organised for discharge.



To review patients on active caseload and amend discharge plans as required seeking senior level of support as required on tasks outside of identified competency.



Carry out assessments using a variety of modalities, this may include physical, cognitive, perceptual and psychological intervention, recording all outcomes and liaising with community services to support discharges.



To identify and order the equipment required to support discharge for those patients case managed by IDT.



To effectively communicate the discharge process and plans to patients / families and carers, leading on discharge planning meetings as required.
