Job Summary


The Care Coordinator will oversee day-to-day care coordination, manage client cases, ensure effective communication between caregivers and clients and provide exceptional customer service.

Duties



Care Scheduling



Coordinate and schedule care visits for clients. Ensure all appointments are booked in a timely and efficient manner, based on client needs and staff availability.

Staff Coordination



Maintain effective communication with care staff to confirm their availability. Ensure accurate shift allocation and provide updates regarding any changes to client care needs.

Client Liaison



Communicate with clients and their families to understand care requirements. Resolve any concerns and ensure client expectations are consistently met.

Record Management



Maintain accurate and up-to-date records of care schedules, client preferences, and other relevant data. Ensure compliance with regulatory and organizational standards. Collaborate with healthcare providers to develop and implement individualised care plans. Monitor and follow up on care plans to ensure adherence to treatment protocols. Assist in administrative tasks such as scheduling, data entry, and maintaining office supplies.

Qualifications



Experience in care coordination and case management Strong communication skills Familiar with home care procedure. Ability to work independently and collaboratively. A compassionate approach towards patient care with a commitment to improving health outcomes.
