About the Role:



We are looking for an experienced Dual Diagnosis Support Worker. The post holder will help to develop and deliver dual diagnosis services, ensuring that high quality services are delivered to service users with complex diagnosis. The successful candidate will work alongside our existing team and clients within the service and will work alongside mental health professionals to support clients with personalised support; to provide a seamless support service. Specific to the role you will be supporting mental health clients transition from a secure care setting to our supported living service and the community with continuation of care and support. You will ensure clients are linked to appropriate services and support networks throughout their journey to sustained wellbeing; and that they have post discharge access to health and social care services in parity with the wider community; reducing the risk of social exclusion, mental health relapse or potential return to secure care settings.

Responsibilities:



Some of the key responsibilities (but not limited to) are as follows:

A good knowledge around different Mental Health disorders is essential for this role in order to ensure you have an understanding of the support our clients require. However, please note all our clients have their own individual support plan that is reviewed and updated regularly, all staff are aware of each clients individual support plan and the support they require. To provide one to one sessions with service users, ensuring that care and recovery planning in relation to Substance misuse is conducted on an individual basis according to the specific needs of the service user. To establish, sustain and disengage from therapeutic relationships with individuals. To support the development and implementation of treatment within the field of dual diagnosis (mental health and substance use/misuse) for service users identified as potentially having a dual diagnosis. Transitioning clients from secure settings to our supported living services and independent living To implement and monitor treatment plans with service users that address their needs in relation to mental health and substance misuse problems. To assess and act upon immediate risk of danger to service users and/or others. To educate and give information to service users and others in the care of the client on issues related to mental health problems and substance misuse. Able to deliver structured psychosocial interventions including harm reduction/minimization strategies, motivational interviewing, CBT programmes, problem solving, behavioural interventions and counselling techniques as negotiated with individual service users.

Qualifications:



Minimum of a Level 3 NVQ in Health and Social Care (Essential) Minimum 3 years experience within a Mental Health role and / or Dual Diagnosis role (Essential) Working with similar clients with diagnoses of Mental Health, Learning Difficulties and Substance Misuse (Essential) Effective IT skills (Essential) Ability to work independently(Essential) Car driver/ability to travel to different sites (desirable)

Pay range and compensation package:



This is a full time position - all of our staff work flexible hours to meet the needs of the business, this includes working weekends and evening shifts. You are entitled to 28 days paid holiday, all training in house, online and external provided when you start your employment with us and throughout the duration of your career with us.
