Job Purpose



To coordinate and enhance dementia-related support across Eastern Devon by building partnerships, developing pathways, and improving local service access for people living with dementia and their carers. This role is focused on linking services, identifying gaps, and strengthening community-based support, especially in rural and underserved areas

Key Responsibilities



Partnership and Coordination



Facilitate the Eastern Devon Dementia Partnership meetings (6 times per year) Represent Eastern Devon at the emerging Devon-wide Dementia Partnership Act as a connector and advocate between local services, primary care, and voluntary sector partners Create mechanisms for collaboration between appropriate agencies

Service Mapping & Development



Maintain and update the directory of dementia-focused services Identify gaps in current provision and contribute to service development and pathway improvement Support and strengthen satellite groups and rural dementia-friendly services, including those in LSOA areas (e.g., Beacon Heath, Wonford)

Primary Care & Community Integration



Work with Primary Care Networks (PCNs) to promote dementia-friendly GP surgeries Develop locality-based, person-centred signposting to ensure local people access local services Explore GP appointment-based access to the Link Worker role

Advocacy & Support



Act as a non-case holding advocate for people living with dementia and their carers Support dementia advisors and link workers with strategic support (not direct caseload management) Enable timely responses for those waiting for formal services, ensuring they can "wait well"

Capacity Building & Training



Facilitate or signpost training for partners via Devon Memory Cafe Consortium and others Support local organisations (including rural groups) to develop new initiatives where services are lacking

Funding & Sustainability



Identify and apply for funding in the prevention and early intervention space Work collaboratively on regional funding bids and initiatives that enhance dementia support

Essential Criteria



Experience working in health, social care, or community development Strong understanding of dementia and its impact on individuals and communities Excellent interpersonal, facilitation, and partnership-building skills Knowledge of voluntary, community and statutory services in Devon Ability to work independently across a wide geographic area Excellent organisational and communication skills (written and verbal)

Desirable Criteria



Experience working within or alongside Primary Care Networks Familiarity with service mapping or directory maintenance Experience in fundraising or bid writing Knowledge of rural health inequalities
