This role is an exciting opportunity within NCL Home Treatment Team. You will be a member of the CYPMHS NCL Home Treatment Team, working to support young people's voices to be heard to improve services.
The Band 6 Educational Practitioner will work as part of the multidisciplinary CAMHS Home Treatment Team (HTT) across NCL, focusing on supporting children and young people in mental health crisis who require targeted educational support. This role bridges clinical and educational systems, providing direct input to schools and colleges, co-developing reintegration plans, supporting care coordination, and promoting educational engagement for children at risk of school exclusion or disengagement due to mental health needs. The Educational Practitoner will support the reintegration of young people into educational settings. They will work closely with schools, colleges, and alternative provisions to develop strategies for supporting young people with mental health challenges in the classroom. The therapist will also offer psychoeducation and tailored interventions that promote academic resilience and attendance. This role bridges health and education, ensuring holistic care.
Provide direct support to CYP and families to promote reintegration into education post-crisis
Liaise with schools, virtual school teams, SEN coordinators, and local authority education reps
Support EHCP-related care planning and attend relevant multi-agency meetings
Develop individualised educational care plans linked to clinical recovery goals
Work alongside Family Ambassadors and Clinical Team to offer system-focused support
Advocate for reasonable adjustments and inclusive practices within school systems
Support transitions from inpatient units or crisis services into school
Contribute to CPA reviews and risk management planning
Maintain accurate records and outcome monitoring (e.g. education re-engagement, attendance improvement)
Promote trauma-informed and neurodiverse-aware approaches in education settings
Support the development of culturally competent and inclusive service delivery
