Role
Launching in September 2025, our Transitions Hub will support young people across the Liverpool City Region who have an Education, Health and Care Plan (EHCP) and are working at Entry Level 2 and above, helping them prepare for adulthood, further education, or employment.
At the Transitions Hub, we are dedicated to creating an inclusive and supportive environment for SEND students. We believe in empowering our learners ready for employment and independence through an ambitious and highly individualised curriculum.
As an EHCP & Transitions Coordinator, you will play a vital role in facilitating the creation, implementation, and monitoring of Education, Health, and Care Plans (EHCPs) for young individuals with SEND. You will work closely with various stakeholders, ensuring that the unique requirements of each student are met and that they receive the support necessary for successful transitions between educational providers.

Key Responsibilities
Coordinate and ensure the timely completion of EHCP annual reviews and transition meetings, adhering to statutory requirements and the SEND Code of Practice.
Collaborate with schools, parents, and professionals to develop tailored transition plans, fostering smooth transitions and continuity of support for students.
Provide administrative support for the Transitions Hub Team, overseeing tracking of additional support and costs.
Work closely with educational staff to gather evidence for EHCP reviews and monitor progress toward set outcomes.
Attend annual reviews and facilitate the transition of students to their next destination, whether further education or supported employment.
Maintain effective communication with students, parents, and carers regarding appointments, assessments, and general enquiries.

Criteria
Experience of interpreting ad implementing the SEND Code of Practice
A sound understanding of Education, Health and Care Plans (EHCPs) including legislation, EHCP processes and responding to EHCP outcomes
A level 2 or equivalent qualification in Maths and English
Knowledge and understanding of the challenges and demands of working with people with SEND and the challenges they face when transitioning from one education provider to another.
Experience of organising large scale meetings or events involving attendees from a number of organisations.
Experience working with databases / information systems.
Excellent written and oral communication and inter-personal skills in order to work effectively with students, colleagues and stakeholders.
