Temporary, Part time, 22.5 hours per week

One Part Time Enablement Co-ordinator is required with the above service on a temporary basis, working 52 weeks per year, on a pro rata basis, including Bank Holidays, some weekend and out of hours working.

This position has become available due to support being required to alleviate winter pressures on the trust.

The service is currently under a restructure and gives an exciting opportunity to anyone who enjoys working in a busy team and enjoys a challenge.

The successful full-time candidate would be working borough wide, alongside health and social care colleagues, to ensure a seamless discharge pathway providing safe and effective service delivery to the vulnerable people of Rotherham.

The successful part-time candidate would be working in the transfer of care Hub each Monday, Wednesday & Friday.

Completing support plans, ensuring that these are factual, up to date and reflect the current needs and goals of the customer.

Ensuring that customers health and medication needs are clearly documented, and In line with Enablement's policy and procedure.

Documenting information on LAS (Electronic data base) regarding customers service start date and time and ensuring plans are updated and reviewed as changes occur.

Ensuring that referrals are made to outside organisations if required. IE Rother care/ Repairs/ assistive technology/ Occupational therapy ETC.

Liaising with other Enablement co-ordinators in the team to ensure sufficient capacity available to meet the needs of the individual.

Engage with other staff, managers as appropriate for the development, implementation, monitoring and quality assurance of the systems required to deliver care service.

Maintaining rotas to ensure sufficient workers available to meet capacity. Keeping within the constraints of the budget by monitoring capacity and staffing levels.

Dealing with the services Fleet vehicles used to deliver service across the borough.

The line management of Enablement workers, completing PDR, supervision and following the sickness procedures as outlined in HR policy document.

Under the direction of the Registered Manager/ Deputy Manager take a leadership role around effective communication across the workforce.
