Core responsibilities Clinical Maintain and apply their nursing knowledge to a range of contexts. Deliver expert nursing assessments and manage complex care needs of care home residents within Weald PCN Oversee chronic disease management including medication reviews and health promotion tailored to care home populations. Act as the named nursing lead for care home services across Weald PCN, advocating for patient-centred care. Provide support to practices with nursing cover during periods of extended absence.
Utilise an understanding of the health and care policy, systems and structures, implications of changing population needs and the political, social, economic, legal, technical, and professional context of healthcare and respond to issues that they encounter. Apply knowledge and skills to a range of professionally challenging situations, within the team. Education Provide clinical support and guidance to care home staff to improve patient safety and health outcomes. Act as a role model for this level of nursing. Assess learning needs and deploy a range of teaching and learning strategies and resources, to support the development of people. Advocate for learning and development of the workforce, with effective succession planning. Participate in the supervision, evaluation and assessment of nurses aspiring to work at an enhanced level. Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate. Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge. Contribute to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers.
