Job Overview


We are seeking a dedicated and proactive Family Outreach Worker to join our organisation. The Family Outreach Support Worker is responsible for providing direct support to families facing challenges with housing insecurity and help connecting them to appropriate community resources and support. You will be working with a caseload of families to support each family to stabilise their situation, address their individual needs and assist them to maintain and become confident in their tenancy.

The role will provide support through home visits and interagency working to ensure a holistic approach to the required support needs. The role will help families in building life skills and enhancing resilience, as well as accessing services such as health care, financial assistance, education, mental health, child services, and any other required support systems.

The Family Outreach Support Worker will also be responsible for recording any repairs required in the properties and liaising with contractors to ensure the work is complete in a timely manner.

Staff will also be involved in an out of hour's on-call rolling rota.

Candidates must have prior experience in supporting individuals/families with different needs and lone working.

Duties



Understand and implement a smooth transition for families into the outreach service and complete all relevant paperwork. Ensure that support meetings are conducted in a caring, considerate, efficient and professional manner at all times. Liaise with both the Manager and the Local Authority Housing Options Team for families both entering and leaving the service. Liaise regularly with all agencies involved with the families' support. Work closely with each family on caseload, on a one to one basis, to implement an agreed support package which addresses their needs and goals. Refer to appropriate specialist agencies for any identified needs and, where appropriate, accompany to engage with statutory and voluntary agency meetings. To ensure each family is given the skills and tools to ensure they are a good neighbour and are able to engage in their local community. To hold a central repairs log for all of the Forever Homes properties. To ensure all repairs are recorded and repair work is booked in using approved contractors in a timely manner according to the rating of the repair required. To encourage the building of strong relationships and support networks. To ensure that at exit from the service, each family has an appropriate package on on-going support.

Pay: 28,149.00 per year
