We are seeking a Rehabilitation Support Worker to provide an exceptional service for our client in Rotherham. This is a rewarding opportunity to be a part of our clients rehabilitation journey and support our client to achieve as much independence as possible. You will be working closely within a team helping to ensure that a continuity of care is provided to our clients.

We recognise the hard work, flexibility and commitment of our people so we offer a wide range of employee benefits, such as a thorough induction programme, on-going training to develop new and existing skills and a great employee Rewards scheme, where you will be able to receive a variety of discounts from major retailers.

About The Role

We are looking for a dedicated and compassionate support worker to provide support to a 29-year-old male with a traumatic brain injury and associated cognitive and physical challenges. This is a rewarding role where you will play an important part in supporting his safety and wellbeing as he transitions from a neuro-rehabilitation setting to living independently in the community.

The client is independently mobile over short distances but requires support with transfers and mobility within the home, including assistance to and from the bathroom and shower. Once seated, he can manage his personal care independently. He does not require any medication support. Your role will include prompting and guiding him through his routine and providing support. This may include tasks such as preparing a light snack or helping to cut food.

Support will be provided on a 24/7 basis to ensure the client has continuous access to assistance throughout the day and night. This is especially important given his vulnerabilities, including a risk of falls, reduced insight, and non-verbal communication. His care needs will be reviewed regularly and adjusted as appropriate.

You'll also support him in the morning to get ready for the day--prompting him with tasks, assisting with mobility, and helping ensure he is up and ready on time. Although he has not displayed recent behaviours that challenge, there is a history of unsettled behaviour, particularly during transitions. A calm, empathetic, and non-judgemental approach is essential, along with experience in managing risk and supporting individuals with communication and cognitive impairments.

Your main attributes will be friendly, approachable and have empathy towards your client treating them with respect and dignity at all times. We support vulnerable people to live as independently as possible, by building their confidence and skills to realise their potential.

This post will be subject to a Disclosure and Barring Service Check
