Case Management supports people who have experienced serious, life-changing injuries so they can live as independently and fully as possible.
At CCM we don't just provide care - we coordinate everything that our client might need to rebuild their life. This includes: Health and rehabilitation
Emotional wellbeing
Education and work goals
Daily living support
Therapies and equipment
CCM is not a care agency. We guide and manage the care and support teams who are employed directly by the client (via a solicitor or Court of Protection Deputy). Support workers work with one client, based on the client's specific needs.
