Key Responsibilities & Duties
Works as part of the Care team
Creating and managing Care Plans
Carryout Spot-checks and Face to face supervisions with carers
Support the continuous development and wellbeing of carers
Maintain proper customer care documentation
Carryout care assessments when and if required
Ensure Unite Care Group provides safe, effective, timely and high quality care
Check the quality of care notes and identify any gaps for improvement
Work well with individuals that use our service.
Prior to each service commencing, carry out with the people we support (and/or their chosen representatives) an assessment of what the customer needs are and risk assessment
Create Care plans tailored to the customer care needs and keeping the care plans up to date
Keep all information about customers and their families secure and confidential
Conducting regular reviews with the people we support
Work well with staff
Give care assistants appropriate information and guidance to effectively and safely provide care and support as agreed in the care and support plan.
Work with the Unite Care Group management team to appropriately match care assistants to the people we support.
Attend the people we support's first call to introduce the care worker.
Report changes to care schedules without delay to the coordinator, person we support and care assistant.
Support, supervise and assist with training of care assistants
Take part in the out of hours emergency on-call rota as and when required.
Quality improvement
Investigate matters relating to the quality of the service and implement improvements
Monitor the performance of care assistants and carry out spot checks at the customer's home to ensure the service is being delivered in line with company procedures
Apply Unite Care Group policies and procedures at all times and make sure that accidents and incidents are recorded, reported and acted upon
Work effectively with the people we support, their families and representatives, other social and health care professionals and staff from Unite Care Group to deliver high quality homecare services.
Your Experience and Suitability
Work flexibly to meet business needs
Have a full, clean UK driving license (essential)
NVQ level 2/3 in Health and Social Care qualification
Confident working independently or as part of a team
Lead staff and individuals we support meetings
Lead and participate in training activities, appraisals and development meetings

Pay: Up to 28,000.00 per year
