As a F ield Care S upervisor, you will be responsible f or the ongoing support of both C aregivers and service users, ensuring deliver y of the high est quality care service s and living up to the excellent company reputation in the community. This role will require a great deal of flexibility and you will need to be focused on achieving the company goals and targets .



Field Care S upervisors are expected to spend the majority of their working time in the community carrying out their primary functions, but their role will typically also entail a certain amount of office-based administrative work.

Providing hands on expertise in the delivery of high-quality person centred care to service users in their own homes in accordance with their support plan . Attending care visits to cover holidays, sick absence, and in an emergency.



Carry out initial risk and needs assessments in respect of individual service users, either in the service user's home or, where required , in other settings such as hospitals or residential/nursing homes. Ensure personalised care planning is in place and true to the service user's needs and requirements



Work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support.



Carry out regular reviews of individual services, including repeated and renewed needs and risk assessments.



Promoting service users health and wellbeing, independence and enabling them to live their life to the fullest.



Carry out the on-site observation and evaluation of C aregiver s ' practice as part of routine supervision s , in response to particular performance concerns, or as otherwise directed .



Ensure outstanding compliance across the business and to support the business in achieving a minimum of "Good" in CQC inspections. Ensure all services are compliant with legislations, CQC standards, regulations and company and local authority's requirements.

Main Duties and Responsibilities



The role of Field Care S upervisor is working with the Registered M anager or their delegate to ensure the operations have outstanding compliance .



You will be required to work on shift pattern, which is four days working and two days off.



Providing hands on expertise in the delivery of high-quality person centred care to service users in their own homes in accordance with their support plan . Attending care visits to cover holidays, sick absence, and in an emergency.



Taking the lead within care visits, ensuring that you p rovide a full introduction for C aregivers and service users.



Carry out initial risk and needs assessments in respect of individual service users, either in the service user's home or, where required , in other settings such as hospitals or residential/nursing homes. Ensure personalised care planning is in place and true to the service user's needs and requirements



Work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support.



Work with Care C oordinators to support the planning of realistic, effectiv e C aregiver rotas, with due regard for the particular needs of the individual and the particular skills and attributes of the C aregiver .



Carry out regular reviews of individual services, including repeated and renewed needs and risk assessments.



Continually review and improve processes to ensure the most effective and efficient service is being delivered . Taking action to make improvements if necessary.



On-board/s hadow new C aregivers in the field.



To assess the competency of C aregivers completing th e care certificate to meet the c ompany's recruitmen t needs. Ensuring all C aregiver s are competent to carry out their job roles.



To ensure training needs of all C aregivers are met effectively - implement, identify and provide for on-going training .



Act on feedback/concerns from service users and communicate appropriately with the rest of the business.



Monitor and assess all service users for customer satisfaction.



Promoting service users health and wellbeing, independence and enabling them to live their life to the fullest.



Treating service users with dignity and respect, communicating effectively to meet their diverse needs .



Maintaining appropriate records to meet the needs of the service, observing and promptly reporting changes or concerns in your service user's circumstances and condition , and act appropriately when these changes arise .



Ensuring all service users' and C aregivers paperwork and files are up to date .



Ensuring all confidential information is held securely.



Update People P lanner/ M obizio rostering system with outcome of support plans/ r isk assessments/reviews / caregivers supervisions and spot check s



Prepare files for new initial referrals .



Carry out the on-site observation and evaluation of C aregiver s ' practice as part of routine supervision s , in response to particular performance concerns, or as otherwise directed .



Mon itor, supervise and mentor all C aregivers.



Provide support to our C aregivers in their day to day job .



Promote a safe working environment and safe systems of work in respect of C aregivers in the field.



To have the knowledge and ability to model good practice whilst being prepared to challenge poor performance .



Understand and follow accident/incident, complaints and safeguarding reporting procedures .



Ensure outstanding compliance across the business and to support the business in achieving a minimum of "Good" in CQC inspections. Ensure all services are compliant with legislations, CQC standards, regulations and company and local authority's requirements.



To act in a professional manner.



To report any areas of concerns to the R egistered M anager.



T o participate in the growth and development of the business, achieving targets and delivery within budgets .



Act on own initiative and exercise decision making skills, as far as possible and wherever appropriate .
