Urban Care Community Limited is an introductory company that connects Clients and Care Professionals to deliver cost-effective and better care. We are currently seeking a workforce requirement for a highly skilled and compassionate Field Care Supervisor.

Reporting: Quality Head
Qualification: Level 3 Health & Social Care or Equivalent
Experience: Minimum of 1 year experience as Field Supervisor or similar roles
Location: Ilford however visit the service users at their address in the Community
Work Schedule : Day & Night shifts every alternate weeks.

Position Overview:



We are seeking a dedicated and experienced Field Care Supervisor to join our clients team. The Field Care Supervisor will play a pivotal role in ensuring the delivery of high-quality care services to our clients. This role involves overseeing a team of care worker, coordinating client care plans, and maintaining excellent communication between clients, care worker, and the management team. The ideal candidate will possess strong leadership skills, exceptional organizational abilities, and a genuine passion for enhancing the well-being of our clients

Responsibilities:



-Client Care Coordination: Collaborate with the care management team to assess client needs, develop personalized care plans, and ensure they are executed effectively by care workers.

-Care Worker Training: Conduct training sessions and orientations for new care worker, ensuring they are well-prepared to provide the highest level of care.

- Quality Assurance: Perform regular home visits to assess the quality of care being delivered, address any concerns, and implement corrective actions as necessary.

- Client Communication: Maintain open and consistent communication with clients and their families, addressing any questions, concerns, or changes in care plans.

- Scheduling: Coordinate care worker schedules, ensuring optimal coverage for all clients while considering care worker availability and preferences.

- Mediation: Address and resolve conflicts or issues that may arise between care worker and clients, fostering a positive and supportive environment.

- Documentation: Maintain accurate and up-to-date records of client care plans, care worker assignments, and all interactions for reporting and compliance purposes.

- Continuous Improvement: Identify opportunities for process improvement and implement best practices to enhance the overall quality of care services.

- Emergency Response: Be available to respond to emergencies or urgent situations, providing guidance to caregivers and ensuring client safety.

- Reporting: Provide regular reports to the management team, highlighting key performance indicators, challenges, and achievements.

Qualifications:



- Proven experience in a supervisory or leadership role in the healthcare or home care industry.

- A background in nursing, social work, or a related field is preferred.

- Strong understanding of care worker best practices and client-centred care.

- Excellent communication and interpersonal skills to interact effectively with clients, caregivers, and the management team.

- Exceptional organizational skills with the ability to manage schedules, coordinate care, and document accurately.

- Compassionate and empathetic approach to client care and caregiver management.

- Proficiency in using technology, including scheduling software and electronic health records systems.

- Valid driver's license and reliable transportation for home visits.

Job Types: Full-time, Permanent

Pay: 26,000.00-28,000.00 per year
