Based at: Connex office in Buxton, but with extensive working out in the community

Reporting to: Heads of Department - Connex Care & Support (CYP) and (OA)
Purpose
To liaise with the office team, Connex care staff and our service users to ensure that the care and support we provide in the community is of high quality and appropriate for the service user's needs.
Support the office team in ensuring the smooth running and delivery of our Care & Support Service.
Duties & Responsibilities
1. Play a key role in the development of good community care practices and procedures and ensure CQC compliance.
2. Visit service users and undertake assessments of task and risk, incorporating this information into the service user's care plan.
3. Introduce support workers to service users and monitor and review ongoing packages of care, undertaking spot checks, assessments and review visits as required.
4. Attend and contribute to case conferences or reviews as required.
5. Ensure that client/staff files are maintained and updated as required.
6. Respond to staff and client rota changes and cancellations, rearranging staff cover as appropriate.
7. Answer telephone and email enquiries, ensuring messages are correctly recorded and actioned.
8. Carry out risk assessments regarding care provision and ensure the working environment is safe for carers and Service Users, identifying any mitigation measures required.
9. Ensure that Service Users' care plans held in the client's home are completed accurately and in a timely fashion by their care staff.
10. Plan, organise and attend group trips/outings for service users.
11. In liaison with management, identify, book and record care staff training as needed.
12. Represent and promote Connex's Care & Support services at networking events.
13. Assist the management staff in undertaking one to one supervisions of care staff.
14. In conjunction with other colleagues provide out of hours phone cover for the service when required.
15. Undertake any training deemed necessary for the post.
16. Undertake any other duties as appropriate.
Person Specification: Field Coordinator - Care & Support Service (CYP and OA)
1. Essential Qualities
1.1 Previous experience of working in a busy office.
1.2 Have a good working knowledge of the care industry and CQC requirements.
1.3 Good organisational skills.
1.4 A Team player with good interpersonal skills.
1.5 Good numeracy and IT skills.
1.6 Good written and oral communication skills.
1.7 Good recording and monitoring skills.
1.8 Self-motivated, able to work on own initiative.
1.9 Able to take responsibility, organise time efficiently and handle conflicting work pressures.
1.10 Must respect the confidentiality of client information.
1.11 Driving licence and use of a car
2. Desirable Qualities
2.1 Hold a minimum of QCF (NVQ) Level 3 in Health and Social Care, and if not, be willing to achieve this once in post.
3. Flexibility
3.1 Ability to work flexible hours as some occasional weekend and evening working will be required, including providing emergency phone cover and covering support calls.
3.2 This is a small office and therefore we require all our staff to be flexible in what they are willing to do.
4. Essential Requirement
4.1 The post holder must be willing to obtain an up to date Enhanced DBS Certificate
