A clinical nurse specialist position with specialist theoretical and practical knowledge in the care of older adults has become available .
The purpose of this role is to provide specialist nursing care, advice, guidance and leadership as a core member of the Acute Frailty Multi-Disciplinary Team .
Support the development and delivery of acute frailty services which will be hospital based but involves collaboration with primary and community services and third sector.
Assess, plan, implement and evaluate evidenced-based nursing care.
Provide specialist frailty advice and excellent clinical practice.

Work as a skilled autonomous practitioner in the identification, assessment, management and treatment of frail older adults in the acute care setting, with a focus on admission avoidance and facilitating timely discharge where admission is required.
Provide specialist education and training to Staff and promote a frailty focus in all areas of the hospital where older adults receive care and treatment.
Work collaboratively with health and social care services across primary, community and acute care services, including third sector parties to establish an integrated and coordinated approach to the management of frail older adults.
Demonstrate the Health Board's values and ensure that others demonstrate the same values.
