Salary:

185.40 per session Tenure: Permanent Hours: One-Two sessions per week Main duties of the job You will undertake a variety of duties, mostly offering support and advice to System providers regarding vulnerable patients in Community as part of an MDT, undertake best interest discussions with patients and family members and help co-ordinate a response to support people in need to stay home whenever possible. More rarely you may be needed to visit a complex frail patient at home or undertake telephone consultations. Make professional, autonomous decisions in relation to presenting problems. Assessing the health care needs of complex patients with undifferentiated and undiagnosed problems, highlighting which services would support them best to stay in their home setting and independent whenever possible.



Cover the clinical management of patients as appropriate including initiating investigations, reviewing results, making referrals to other providers/services as appropriate. Record clear and contemporaneous computerised consultation notes to agreed standards. Compile and issue computer-generated acute and repeat prescriptions. Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).



Work with and support other members of the MDT, including but not limited to: social workers, therapists, social prescribers, voluntary support workers, frailty nurses, dieticians, UCR paramedics and other Frailty GPs. Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, British Geriatrics Society and local NHS trust. Participate in national frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.
