Responsibilities
Dispense medications accurately and efficiently, ensuring compliance with legal and regulatory requirements.
Provide comprehensive medication counselling to patients, addressing their questions and concerns regarding prescriptions.
Conduct medication reviews to identify potential interactions or contraindications, ensuring patient safety.
Collaborate with healthcare teams to develop and implement patient care plans that include medication management.
Monitor patient responses to medications and adjust treatment plans as necessary in consultation with prescribers.
Maintain accurate records of prescriptions, patient profiles, and medication inventories.
Educate patients on proper medication administration techniques and the importance of adherence to prescribed therapies.
Stay updated on new medications, therapies, and industry best practices to provide the highest level of care.
Qualifications
A valid pharmacist licence in accordance with local regulations.
Proven experience in a pharmacy setting; experience in an ICU is preferred.
Strong knowledge of anatomy and pharmacology relevant to medication administration.
Excellent communication skills, with the ability to convey complex information clearly to patients and healthcare professionals alike.
Strong organisational skills with attention to detail in managing multiple tasks effectively.
