About the Role
You will work as an integral part of our multi-disciplinary team, providing evidence-based nursing care to patients with a range of health needs across all age groups. The successful candidate will be responsible for both chronic disease management and acute patient care within a primary care setting.
Key Responsibilities
Management of long-term conditions (e.g. diabetes, asthma, COPD, hypertension)
Dietary advice
Urinalysis
Childhood and adult immunisations & vaccinations
Cervical cytology (smear tests)
Wound care, dressings, and suture removal
Travel health advice and vaccinations
Health promotion and screening
NHS health checks
Contraceptive advice and support (if trained)
Patient education and self-care promotion
Supporting the practice in meeting
QOF (Quality and Outcomes Framework)
indicators
Providing support to GPs and the wider clinical team
Keeping up to date with all mandatory and role-specific training
Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
Essential Criteria
Registered Nurse with valid NMC PIN
Previous experience in a primary care or community setting
Excellent clinical, communication, and organisational skills
Able to work autonomously and as part of a team
Fully compliant with
Infection Prevention & Control (IPC)
protocols and a sound working knowledge of
Infection Control Procedures (ICP)
in primary care
Experience supporting QOF-related work, chronic disease management, and patient recalls
Maintenance of equipment and stock relating to patient care
Attend and participate in practice meetings as required
Confident using IT systems for clinical documentation and patient records
Commitment to maintaining up-to-date training and ongoing professional development
Desirable Criteria
Chronic disease management qualifications or experience
Cervical cytology trained
Immunisation training
Travel health qualification
