Job Summary
The post holder will be responsible for the clinical coding of relevant information into patients' electronic healthcare records on the clinical system, adhering to the standard SNOMED CT codes and relevant organisational policies.
The post-holder will support staff with the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.
To be responsible for the read coding of relevant information into patients' electronic healthcare records on the clinical system, adhering to the organisation's coding policy. In addition, the post-holder will support the multi-disciplinary team, providing administrative support as required, in line with the needs of the organisation.
Duties can also include, but are not limited to, supporting the administration and receptionist teams with patient registration, booking appointments, and processing of information (electronic and hard copy) in a timely manner and in accordance with current policies.
The post-holder will be an integral part of the general practice team.

Duties
The following are the core responsibilities of the Receptionist. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. Review records and accurately produce a summary of the patient's medical history
b. Accurately SNOMED CT code data on the clinical IT system
c. Review progress of summarising, ensuring practice targets are achieved
d. Review all clinical letters, ensuring they are processed appropriately in line with extant protocols
e. File and store records as required
f. Photocopy documentation as required
g. Action all incoming correspondence, emails and mail in a timely manner
h. Scan patient related documentation and attach scanned documents to patients' healthcare records
i. Correctly input data into patients' healthcare records
j. Answer incoming phone calls, transferring calls or dealing with the caller's request appropriately
k. Conduct system searches as requested
l. Support all clinical staff with general administrative tasks as requested
m. Maintain a clean, tidy, effective working area at all times
n. Be an integral part of the general practice team
o. Act as a chaperone as required
p. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
q. Undertake all mandatory training and induction programmes
r. Contribute to and embrace the spectrum of clinical governance
s. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
t. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action
u. Inputting and extracting data as required to support practice reports, audits and to capitalise upon both patient safety and QOF income
v. Scanning patient related documentation and attaching scanned documents to patients' healthcare records
w. Highlight all timely actions, changes to patient care and management to the relevant clinician
