We are looking for a motivated Care Navigator / Patient Services Team member to work in a busy NHS GP Surgery.
Hours of work will be Monday to Friday as part of a shift pattern ( 7.30-3.30, 8-4pm and 11-7pm)
We are operating a total triage model of managing our patient population which means our trained and dedicated care navigators are responsible for answering calls and running the switchboard. Responsible for navigating the calls and taking a history from patients to allow the medical team to triage the call.
You will need to be articulate, calm and empathetic to patients. Must have a keen eye and pay attention to detail.

Greeting patients and visitors in an efficient and courteous manner.
Provide an effective and polite telephone enquiry service, using Practice care navigation pathways.
Proactively communicate information between relevant patients, doctors and professionals.
To provide full support to the Team Leaders ensuring an efficient and effective clerical and administrative service to the Practice.
The maintenance of an effective filing and recording system for patient data and practice documents.
Duties
Your duties include but are not limited to:
u Dealing with emails on a timely basis effectively, distributing them to the correct person or service and maintaining the email account
u Greet patients and visitors to the practice in accordance with Practice standards and procedures.
u Respond or redirect all patient and visitor requests accordingly.
u Answer incoming telephone calls, ensuring calls are documented and redirected accordingly per practice care navigation pathways
u Allocating of appointments correctly
u Ensure computerised appointment and patient systems are up to date and all data is accurately input.
u Distribution of completed prescriptions
u Photocopying
u Shredding
u Filing
u Maintenance of files
u Processing of information and assisting patients as required
u Relaying information both verbally and written to patients i.e test results
u Checking your personal NHS e-mail account throughout each shift
u Distributing information
u To help to complete QOF lists
u Palliative care services
u To attend PLT sessions and engage with staff meetings
u Dealing with messages and queries for the clinical team
u Liaising with external members of the team such as secondary care, ARRS and community service providers.
u To maintain confidentiality at all times
u To maintain the correct and current Data Protection Standards at all times
u Accurately enter patient information as required
u To update patient details as needed
u To log onto clarity Team net on shift to keep abreast of changes/documentation.
u File patient's records. Also to retrieve and refile hard copy records appropriately
u To act on behalf of the clinical team and practice manager as and when required
u To sign in and out of the building at the start and end of each shift
u To complete statutory and mandatory training
u To report any issues, concerns or complaints to the Practice Manager first as a matter of priority.
u To record, document and process medical reports and subject access requests
u To complete other administrative duties as requested by the line manager
