Care Coordinator

About the Role:
As a Care Coordinator, you'll play a key role in helping patients navigate health and care services. Working closely with GPs, clinical staff, and external partners, you will ensure patients receive timely, personalised, and coordinated care.
The Care Coordinator role is seen as a critical and evolving post to support the Enhanced Health in Multi-Disciplinary Teams within the locality to deliver effective, co-ordinated care for all patients including vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, ED attendances or out of hours care.
Proactively coordinate personalised care and support planning for the most vulnerable people in the community, including the frail/elderly and those living with other long-term health condition
Key Responsibilities:
Coordinate care for patients with multiple health and social care needs
Liaise with multidisciplinary teams (MDTs) to ensure joined-up care
Help patients access services, appointments, and follow-up support
Maintain accurate and up-to-date records
We're Looking For:
Excellent communication and organisational skills
Compassionate and patient-focused approach
Ability to work independently and within a team
Experience in primary care health or social care settings (preferred)
Proficiency in IT systems and Microsoft Office
Educated to GCSE level or equivalent -GCSE Mathematics & English (C or above)
Desirable:
Knowledge of the NHS and local health care system
Experience with SystmOne
Care coordination or care navigation experience
