We are a practice consisting of 6 GP Partners, 4 salaried GP's, 5 Practice Nurses, 4 Health Care Assistants, and a lovely admin team. We have around 9,000 patients. We operate from a large multi-purpose building in Eccleshill, where we also have Physiotherapists Social Prescribers, Ultrasound, Epilepsy and memory clinics and the District Midwives. We have a strong training focus in practice, training 6 GP registrars, FY2 doctors, medical students, advanced nurse practitioners, and nursing students. Job Overview:
The Practice Lead Nurse provides clinical care and support to patients under the direction of the GP's and the Practice Manager. Whilst supporting and managing the nursing team of 4 nurses and 4 HCA's.
Key Nursing Responsibilities:
Patient Assessments
: Conduct health assessments, including taking medical histories, monitoring vital signs, and performing routine screenings (e.g., blood pressure, blood tests, ECGs, Spirometry). Supporting the achievement of QOF targets
Chronic Disease Management
: Provide ongoing care for patients with chronic conditions such as diabetes, asthma, hypertension, and COPD, including monitoring, education, and support.
Ideal candidate will be confident in diabetes management, blood glucose monitoring, patient education, and supporting lifestyle modifications.
Immunisation & Vaccinations
: Administer routine immunisations and vaccinations to children, adults, and travellers.

Health Promotion & Education
: Offer advice and education on a range of topics, including smoking cessation, weight management, sexual health, and general wellness.
Wound Care
: Provide treatment for minor injuries and wounds, including dressing changes, suture removal and any other necessary procedures.
Family Planning & Sexual Health
: Offer family planning services, including contraceptive advice, STI testing, and sexual health advice.
Minor Illness & Injury Treatment
: Assess and treat minor ailments such as infections, skin conditions, and injuries.
Medication Administration
: Administer prescribed medications, injections, and other treatments as needed.
Assist with Medical Procedures
: Assist GPs in performing medical procedures and examinations, including taking blood samples and urine tests.
Patient Record Management
: Ensure accurate and timely documentation of patient care, including medical records and treatment plans, using electronic health systems.
Clinical Audits
: Participate in regular clinical audits to ensure high standards of care and identify areas for improvement. Including Infection control audits, fridge checks and managing the drugs log.
Team Meetings:
Actively participate in the variety of practice meetings.
Mentor, support and develop colleagues and students:
Support the variety of students we have on placement or training in practice.
Collaboration & Teamwork
: Work closely with GPs, other nurses, healthcare assistants, and administrative staff to ensure effective patient care delivery.
Emergency Response
: Provide initial assessment and care for patients in urgent situations before the arrival of a GP or paramedics, if required.
Conduct annual appraisals for Practice Nurses and HCAs. Provide continuous feedback and support professional development. Mentor, Support and Develop Colleagues and Students: Provide leadership, mentorship, and supervision to Practice Nurses, HCAs, and students on placement or training within the practice.
Training & Induction:
Identify training needs, coordinate mandatory and specialist training, and oversee the induction of new clinical staff.
Nursing Team Leadership:
Provide day-to-day clinical leadership and act as a point of contact for clinical advice and support within the nursing team.
Revalidation Support:
Support nurses in preparing for NMC revalidation, including portfolio guidance, reflective discussions, and evidence gathering.
HR Liaison:
Work alongside the Practice Manager on HR matters such as absence management, performance reviews, and ensuring compliance with employment policies and procedures.
Protocol & Policy Development:
Lead the development, review, and implementation of clinical protocols and standard operating procedures in collaboration with the Practice Manager and GP team.
Quality Improvement:
Drive quality assurance initiatives, monitor care outcomes, and implement improvements based on audit findings and patient feedback. Team Meetings: Actively participate in and contribute to a variety of practice meetings. Represent the nursing team and share updates or feedback as needed
Qualifications & Skills:
Registered Nurse
with a current NMC (Nursing and Midwifery Council) registration.
Experience
in general practice is ESSENTIAL.
Management/ leadership Experience
Confident in Diabetes Management
: (Desirable) Have the ability to confidently manage diabetes care, including patient education, monitoring, and support for insulin therapy and lifestyle changes.
CPD (Continuing Professional Development)
in relevant areas such as chronic disease management, immunisations, or family planning.
Good Knowledge
of clinical guidelines and protocols related to primary care nursing.
Excellent Communication Skills
: Ability to communicate effectively with patients, families, and the healthcare team.
