The Health Care Support Worker (HCSW) plays a vital role in providing compassionate care and support to individuals in the primary care setting. You will be seeing patients to undertake clinical duties such as phlebotomy, ECGs, blood pressures, diabetic foot checks and gathering information for their long term condition reviews.
We are looking for someone who can work either full or part-time (18-35 hours). The role will be working across our two sites (Ashgrove Surgery CF37 2DR and Penrhiwceiber Medical Centre CF45 3SQ)
The HCSW will work closely with healthcare professionals to deliver high-quality care and contribute positively to the patient experience.
Responsibilities
1. Be responsible for the management of a group of patients with regard to Venepuncture, Physical Assessment, Adult Immunisations, Wound Care, Minor Surgery Assisting and supporting the clinical team in Chronic Disease Management, Health Screening, Mental Health, Men's and Women's Health, and the management of Emergency Situations.
2. Be responsible for maintaining the standards of patient care set by primary care
3. Be responsible for ensuring that all policies and procedures are observed
4. Be responsible for the support of staff i.e. Phlebotomist, learners.
5. Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client
6. Liaise with the appropriate Clinical Specialists i.e. GPs, Practice teams and Nursing Team, in relation to individual patient requirements.
7. Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.
Requirements
Level 3 HCSW Apprenticeship in Primary Care
Able to support management of long term conditions
Change-management skills and ability to support patients to change lifestyle
Good communication skills, both written and verbal
Ability to communicate difficult messages to patients and families
A valid Enhanced Disclosure and Barring Service (DBS) check is required prior to employment.
