What will the role look like?

The role will support adults to navigate services and find accessible support to reduce their increased contact with urgent healthcare services.


The outreach role will involve meeting with people to carry out holistic assessments and explore options that support them to better manage their own health and wellbeing

We are looking for someone that is passionate about supporting others.


We are looking for someone that pro-actively advocates for the needs of individuals, demonstrating creative and lateral thinking and applying it to their work.


We are looking for someone that positively influences others, whilst being a good listener and being able to communicate clearly and effectively.
