Key Responsibilities:



- Monitor and assess clients' condition during the shift

- Assist clients with personal care tasks, including bathing, and medication administration

- Communicate effectively with clients, their families, and the care team to ensure continuity of care

- Document and report any changes in clients' health or condition to the appropriate healthcare professionals

Required Skills and Qualifications:



- Drive and have access to your own vehicle.



- Prior experience in a similar role, preferably in a home care setting

- Strong understanding of complex care needs and ability to respond.

- Excellent communication and interpersonal skills

- Ability to work independently and part of a team.

- Compassionate and client-centred approach to care
