Health & Wellbeing Coaches play an important role providing extra time, capacity and expertise to support patients to be actively involved in managing their care. They empower people to take control of their Health and wellbeing, supporting them to make choices that are right for them. They consider the whole person in addressing existing issues and encourage the proactive prevention of new illnesses, promoting lifestyle medicine approaches.

Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills and confidence to manage their health and wellbeing whilst increasing their ability to access and utilise community support offers

Health & Wellbeing coaches work independently as well as part of a team, are forward thinking, caring, dedicated, reliable and person-focused individuals who enjoy working with a wide range of people.

Our vision as a PCN is to work collaboratively to deliver the best outcomes for our patients and staff. With a PCN population of just over 51,000 patients, we are recruiting for 1 whole time equivalent health & wellbeing coach who will focus on Bottisham Medical Practice, East Barnwell Health Centre, York Street Medical Practice, Nuffield Road Medical Centre, Arbury Road Surgery and Cambridge Access Surgery.



Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what is important to them, with the aim of: Improving people's knowledge, skills and confidence in managing their condition/s Empowering people to manage their own health and improve their health outcomes Support people in making changes in their health related behaviour Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the health and wellbeing coach role - e.g. when there is a mental health need requiring the patient to be referred to an appropriately qualified practitioner. Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management, modelling the coaching approach in your work. Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the Health and Wellbeing Coach role, how to refer to them, and which patients may particularly benefit from health coaching. Support local health, social care and voluntary sector professionals to make appropriate referrals to the service. Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access. Attend and contribute to team, practice, and PCN meetings and events as required by the service. Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role. Participate in regular health coaching supervision and continual learning. This may include, but is not limited to, any or all the following: Regular contact with service supervisor Refresher training sessions Buddying with peers Peer support sessions 1:1 support from a practitioner with more health coaching experience Action Learning Sets e-learning to revisit or deepen training On-going improvements to systems and processes (Dependant on skills and ability) Contribute to and take part in health coaching train-the-trainer workshops in order to spread health coaching skills to support a mindset shift among staff in how they have conversations, integrating a health coaching approach into how they work with people

Key Working Relationships



The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders within and beyond the organisation. Key relationships will include:

GPs and GP practice staff Patients and their carers Community Specialist Practitioners Community Nursing Staff Care and Residential Home Staff Secondary Care discharge teams Integrated Neighbourhood teams & affiliates

Person Specification



Essential criteria:



Good general education including core level of Maths and English NVQ Level 3 Health Trainer qualification or other relevant professional academic qualification Able to interpret, translate and deliver key messages to patient groups Ability to demonstrate management of own caseload and prioritisation of case work. Excellent communication skills (written and oral) Strong IT skills Clear, polite telephone manner Competent in the use of Office 365 and Outlook Effective time management (Planning & Organising) Ability to work as a team member and autonomously Good interpersonal skills Problem solving & analytical skills Ability to follow policy and procedure Ability to deal with challenging behaviour Able to interpret, translate and deliver key messages to patient groups Experience preparing plans and reporting progress against KPIs and other metrics Polite and confident Flexible and cooperative Motivated Forward thinker Sensitive and empathetic in distressing situations Ability to work under pressure Ability to work independently Disclosure Barring Service (DBS) check Ability to travel between service sites - a valid driving licence and own car is a must

Desirable criteria:



Leadership and/or management qualification (or equivalent relevant experience Experience of working with individuals with long term conditions Knowledge of General Practice EMIS / SystmOne knowledge
