The role will include independently undertaking home visits for the PCN Enhanced Care Team, completing long term condition (LTC) reviews and Frailty assessments. The successful applicant will also help the team with demand for phlebotomy and HCA services.
This role will require an
Enhanced DBS Clearance
. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this, providing the original is shown. Otherwise, you will be expected to complete a new application.
As the role involves patient home visits, you are required to own a vehicle for work and have Business Car Insurance.
Main duties and responsibilities (this list is not exhaustive):
Working in the community with housebound patients under the direction of a qualified nurse.
To collect venous/capillary blood samples without harm to patient or self by following the phlebotomy standard operating procedures.
To obtain such blood samples to a standard fit for laboratory testing ensuring collection, maintaining appropriate files & records as required.
To ensure that all phlebotomy equipment and accommodation is properly maintained and suitable for use.
To competently complete Frailty assessment visits.
To competently complete Long-Term Condition (LTC) reviews.

Dopplers
Wheelchair assessments
Pain assessment forms
Basic observations
Measurement for stockings
Inhaler technique
Collection of prescriptions
Experience of home visiting is essential however further training on frailty assessment and LTC reviews will be provided.

PERSON SPECIFICATION (this list is not exhaustive)
Essential criteria
GCSEs or similar, to include English Language and Maths
Strong communication in English, both written and spoken.
NHS Care Certificate
Ability to deal with distressed patients in an empathic, caring and compassionate way.
To have their own vehicle and hold a clean driving licence to meet the travel requirements of the role. You will require business insurance
Ability to write and maintain accurate clinical records
Have strong IT skills
Good time management skills
Be able to prioritise their day and be familiar with all routine blood tests and understand their clinical importance
To be able to work independently within a team structure and possess excellent communication skills
Demonstrate a flexible approach to work and to follow instructions and guidance given to them
Qualified in phlebotomy.
Experience in Home visiting.
Experience in LTC reviews and basic frailty assessments.
Desirable criteria
NVQ level 2 or Level 3 in Health and Social Care
To demonstrate and be up to date in competency and understanding of phlebotomy technique and sampling conditions
Previous experience of working in a Primary Care setting
Previous HCA or Phlebotomy experience in a healthcare setting
