Were looking for a compassionate and proactive Healthcare Assistant to join our Integrated Neighbourhood Team at Banbury Cross Health Centre. You'll play a vital role in supporting complex, vulnerable, and often housebound patients across the Banbury area. This is a dynamic and rewarding position where you'll manage a dedicated caseload, conduct home visits, and contribute to holistic, patient-centred care.
About our team
Our Integrated Neighbourhood Team is a collaborative, multi-disciplinary team (MDT) including GPs, Care Coordinators, Social Prescribers, and other healthcare professionals within the wider primary care team at Banbury Cross Health centre.
Our mission is to reduce avoidable hospital admissions by proactively managing patients in the community. We identify patients with increasing frailty and address health inequalities within our deprived patient population. We take referrals from health care professionals and proactively review patients post hospital discharge who meet our criteria.
We work in partnership with District Nurses, community therapy services, PML neighbourhood frailty and visiting teams, social workers, Hospital at Home, and Geriatricians to deliver integrated care. We also work with community and voluntary organisations such as Age UK, Dementia Oxfordshire, Cherwell District Council, and the Volunteer Transport Service to ensure our patients receive comprehensive support.
This role may close early if sufficient applications are received.

As a key member of our MDT, you'll:
Deliver care to patients in their homes across Banbury (business-insured vehicle and full UK driving licence required)
Support clinical colleagues with patient reviews and care planning
Perform clinical tasks such as phlebotomy, ECGs, blood pressure checks, and simple dressings
Recognise signs of deterioration and escalate concerns appropriately
Support patients and families with increasing frailty

Attend and contribute to MDT meetings
Maintain accurate patient records and assist with referrals
Uphold infection control standards and contribute to clinical governance
