Were looking for a compassionate and proactive Healthcare Assistant to join our Integrated Neighbourhood Team at Banbury Cross Health Centre. Youll play a vital role in supporting complex, vulnerable, and often housebound patients across the Banbury area. This is a dynamic and rewarding position where youll manage a dedicated caseload, conduct home visits, and contribute to holistic, patient-centred care. 
About our team 
 Our Integrated Neighbourhood Team is a collaborative, multi-disciplinary team (MDT) including GPs, Care Coordinators, Social Prescribers, and other healthcare professionals within the wider primary care team at Banbury Cross Health centre. 
 Our mission is to reduce avoidable hospital admissions by proactively managing patients in the community. We identify patients with increasing frailty and address health inequalities within our deprived patient population. We take referrals from health care professionals and proactively review patients post hospital discharge who meet our criteria. 
 We work in partnership with District Nurses, community therapy services, PML neighbourhood frailty and visiting teams, social workers, Hospital at Home, and Geriatricians to deliver integrated care. We also work with community and voluntary organisations such as Age UK, Dementia Oxfordshire, Cherwell District Council, and the Volunteer Transport Service to ensure our patients receive comprehensive support. 
 This role may close early if sufficient applications are recieved.
