Overview


We are seeking a compassionate and dedicated Healthcare Assistant/Administrator to join our Frailty team. In this role, you will provide essential support to individuals in a variety of settings, including home care and care homes. Your primary focus will be on assisting clients with daily living activities, ensuring their comfort and wellbeing while promoting independence. The ideal candidate will possess strong communication skills and the ability to work collaboratively within a team.

Duties



To work as a member of the Frailty Team to assist with the delivery of the clinical case management for patients unable to attend the surgery. To support the delivery of the Amber Frailty Team service, under the supervision of trained nurses, undertake a broad range of health care activities and duties relating to the care of frail and complex patients and those with long term conditions. To carry out assessment and care plan in accordance with instructions and recognised training received. This will involve some level of decision-making and prioritising workload. To support the Same Day Team visiting patients in their own home and occasional support for GP clinic sessions. Working in partnership with patients, families and carers, primary and secondary care services, statutory organisations and third sector parties. Participating in support and education of patient and carers to promote independence, good health and personal safety. Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process gathering feedback to improve services. Provide and maintain a safe and comfortable environment for all patients and carers and seek further advice from trained staff if an emergency situation should arise. Undertake risk assessments for service users within the home environment. Contribute to audit and data collection as required. Understanding their role in the team and how they help the team achieve its' objectives Participate in regular team meetings and attend practice learning time and other meetings as and when required. Responsible for their own personal development, including keeping up to date with Mandatory training and training relevant to the area of practice. Undertaking a link role within the team as required. To assist with the identification of frail and complex patients and those with long term conditions within the locality, supporting preventative care, screening and patient education. To help people to manage their needs through answering queries, making, and managing appointments. To support patients to utilise decision aids, help create single personalised care and support plans, in line with best practice. Work with the local care homes to provide a single point of access to co-ordinate patient care. Arrange referrals to medical professionals within various clinical disciplines. This will include referrals to dieticians, physiotherapists, speech and language therapy and for memory assessments. Liaise with families and arrange appointments with the Care Home GP. Arranging virtual and physical meetings for health care professionals and family.

Experience



Previous experience in home care or a similar healthcare setting is desirable but not essential. Familiarity with IT systems for maintaining care plans and documentation is an advantage. Strong command of English is required for effective communication with clients and colleagues. A caring attitude towards individuals in need of support is essential for success in this role.
