Job Description

SS9 South PCN is looking for a healthcare assistant (HCA) to join the practice in this vital patient-facing role with both clinical and administrative responsibilities. The role is offered on a part-time basis, for 15 hours per week on Monday, Wednesday and Fridays.
The role will offer you the opportunity to build on your experience, skills and competence, requiring you to work effectively within a multidisciplinary healthcare team. • Assist with patient assessments, basic treatments, and monitoring under supervision.
  • Contribute to the smooth running of urgent and primary care services across GP practices, community settings, and home visits.
  • Provide a patient-centred approach, ensuring dignity, safety, and confidentiality at all times.
  • Carry out clinical observations (e.g., blood pressure, pulse, temperature, oxygen saturation).
  • Perform basic diagnostic tests (e.g., urine dipstick, blood glucose monitoring)
  • Support with wound care, dressings, and removal of sutures/clips (where trained).
  • Assist ECPs during consultations, examinations, and minor procedures.
  • Deliver health promotion, lifestyle advice, and patient education.
  • Accurately document patient information in electronic health records.
  • Support infection prevention and control practices.
  • Accompany ECPs on home visits and provide practical support.
  • Assist with administrative duties, including booking follow-ups, ordering stock, and maintaining supplies.
  • Work collaboratively with the wider multidisciplinary team to ensure continuity of care.
