Lewisham and Greenwich NHS trust has an exciting opportunity for Healthcare Support Worker to join the Adult Community Nursing Service (Twilight Service) delivering services to patients in their own homes in one of our 4 local neighbourhoods.

The job role includes monitoring patients' conditions by taking vital signs and escalates changes in patients' conditions to the district nurse or senior nurse on duty.



Taking blood for testing (Phlebotomy) and administration of insulin following appropriate training and consolidation; perform catheter care and pressure area/ skin integrity checks.



Carrying out simple wound dressings and other relevant nursing tasks as assigned by the senior nurse.



Promotes patient independence through implementing rehabilitation programmes as directed by the multi-professional team



Undertakes health promotion activities with patients and their families as directed by the District Nurse or members of the multi-disciplinary team



Responsible for ensuring up to date stock levels of materials and equipment used by the team



Maintains accurate documentation immediately after nursing intervention in line with Trust policies, guidelines and procedural documents



To use mobile working devices to record and document relevant information



Undertake routine audits as required



The two shift patterns for the role:

08.00 - 16.00 and 12.00 - 20.00 including weekends and bank holidays.



The post holder must have a full current UK driving licence and daily access to a car, as this is essential for the role
