We are seeking a passionate, dynamic and experienced practitioner to join our Healthy Family Team.
Our teams are fully integrated consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners and Community Connectors.
Healthy Family Support
Practitioner
Role
To
work
with
children
,
young
people
and
their
families
, on a one to one or group basis, using a variety of interventions, to identify needs and to achieve positive outcomes.

parents/carers to empower them and to enable them to achieve better outcomes for

children

, to provide the most effective

children
young
people
and
families
.
+ Undertake developmental and
health
reviews of
children
and
young
people
at key stages of
their
life
We are looking for a practitioner who is experienced in
working
with
children in their early stages of life including supporting families in the antenatal period.
0-5yrs
Working in an integrated team to support the delivery of the Healthy Child Programme
Supporting the delivery of Maternal Early Childhood Sustained Home visiting programme (MECSH)
Promoting first relationships and attachment
Promotion of the Baby Friendly Initiative (BFI)
Providing universal and targeted support to families through delivery of group sessions
Working in partnership with individuals and their families to achieve positive outcomes
Deliver evidenced based parenting support to all families
Providing evidence-based interventions with families to support their children to become School Ready and meet age appropriate levels of development
Organising and undertaking Integrated two-year reviews in partnership with the family and the child's Early years setting
To participate in and deliver ECFWS virtual sessions to ensure all families are reached and have meaningful contact and achievable outcomes
To make contact with families where parental conflict has been identified as a 'risk' factor, providing brief intervention support and advice
Participating and leading on health promotion activities in the family hubs and delivery sites or at external venues and events

Essential requirements for the
role
Qualification mandatory for practice -A minimum NVQ Level 3 relating to children and young people and including child development.
Can demonstrate some experience of working with children and young people

+ Able to observe and recognise
child

young
person or
family
's behaviour,
understand
its context and respond to concerns about developmental or behavioural changes.
A valid driving licence and use of a car is essential for this role.

As per NHS guidance this role may require successful applicants to evidence immunisations or immunity for MMR and Chicken Pox. This is due to the clinical focus of some tasks and the requirement to work with vulnerable families, young people and children.
The roles within this service may also require the successful applicant to wear PPE including the wearing of a protective mask and to follow social distancing rules as highlighted in NHS guidelines. Successful applicants will be required to remain compliant with any future mandatory requirements introduced by the Government/NHS.
