We are looking for passionate and driven people who want to join our Coventry-based team to work within the local community. They will be a key part of our team delivering a holistic health and wellbeing service in workplaces and community settings.

The role is ideally suited to people with strong communication skills and those who are confident delivering one-to-one holistic health and lifestyle advice to individuals, including carrying out initial assessments and brief interventions using proven behaviour change techniques.

You will have a wide and varied caseload and will need to be able to prioritise multiple clients' needs via multiple channels.

You will be expected to support with any community engagement to help promote the service and the work we do.

It is hybrid role where you will mix your time of being in the office, out in the community and working from home.

This is a Full-time role. Hours to be covered are Monday - Sunday within the hours of 8am - 8pm. Flexible hours are required to meet the needs of the service including evening/weekend work in agreement with the service lead.

Where this is not required your core hours will be between Monday and Friday 08:30am and 5pm.

You will be required to travel and have a clean driving license and have access to a car at all times.

A clear and satisfactory DBS check will be required for this role.

Key elements of the day to day role:

You will be play a pivotal role in providing and promoting the smokefree generation within the City of Coventry You will primarily be responsible for coaching/supporting current tobacco users to set a quit date and the successful achievement of a 4 & 12 week quit. You will work to agreed targets and objectives as part of the team to ensure that the service delivers what it needs to meet the smokefree generation promise.
You will also enable individuals to change their behavior through our integrated service approach to improve their health working to achieve the following change measures:

achievement of 3%, 5% or 10% weight loss reduction of smoking by > 50% for patients with SMI (non alcohol-dependent) patients reducing drinking by 50% or to within CMO guidelines no of patients increasing physical activity levels to CMO guidelines Communicate with individuals about health and lifestyle improvement. Manage and organise own time and activities to support individuals in the secondary care sector.

What you'll bring



A good knowledge about things that are important to people in the local community.

Understanding of behaviour change.

Experience of working with individuals and groups in communities experiencing health disadvantage / inequalities

Experience in using Zoom, Microsoft Teams, and other digital platforms.

Travel - You will be required to travel to community locations when appropriate to provide clinic cover and support, so own car is preferred.

Experience in using Zoom, Microsoft Teams and other digital platforms.

Essential existing skills/experience



Understanding of behaviour change techniques

IT literate and able to confidently use online methods of communication. You will also need to have a good eye for detail and be accurate with data input and reporting.

Experience of delivery of a wide range of health interventions, including weight management, physical activity, stop smoking advice, alcohol brief interventions.

Knowledge about health and health services need of the local community

Travel - You will be travelling to community locations when appropriate to provide clinic cover and support, so own car is essential.

Full training for all roles will be provided along with a programme of continued professional development. As this post is community-based, post holders will need to be able to independently travel across the Coventry area.

Desirable existing skills/experience



Undergraduate degree in Nutrition, Human Sciences.

A good knowledge of the Coventry area

A good knowledge about things that are important to people in the local community

Experience of working with individuals and groups in communities experiencing health disadvantage / inequalities
