Job Overview


To lead and deliver high-quality care and support to a diverse range of vulnerable individuals within the community, empowering them to achieve positive outcomes, greater independence, and control over their rights and choices. This role is pivotal in ensuring person-centred, aspirational care that upholds dignity, respect, and individual well-being. Collaborating with the manager and multidisciplinary professionals, you will oversee and contribute to services that meet the highest standards of care, fully compliant with RISCA regulations, Social Care Wales guidelines, and contractual requirements. This position is for 30 hours a week.

Responsibilities



Conduct regular performance reviews, including one-to-one meetings, identify performance gaps and create development plans where required for colleagues you have line management responsibilities for. Lead by example to embed a person-centred approach to care and support, assist with personal care tasks, including medication management, in alignment with care plans, that foster independence and dignity. Coordinate daily operations and allocate resources to ensure efficient team performance and rostering that meets the support needs of individuals to appointments, such as hospital or GP visits. Promote health, activity, and inclusion, enabling individuals to feel valued within their communities. Ensure team compliance with organizational policies and procedures and the safety and security of individual. Promptly and accurately report any safeguarding concerns in line with Thrive Group Policies. Monitor adherence to health, safety and other regulatory requirements. Support individuals to safely store and prepare food appropriately in compliance with food safety standards. Undertake regular reviews to ensure care plans remain up-to-date and affective. Provide coaching and mentorship to support colleagues' personal development in maintaining records and delivering support in line with care plans, and risk management documentation. Demonstrate a strong understanding and genuine motivation to assist individuals in completing domestic tasks, fostering their independence and daily living skills. Attend supervision meetings, team meetings and training session, taking responsibility for your ongoing professional development. Respect and support individuals' cultural, religious, and personal preferences in all aspects of care and service delivery. Support individuals' emotional well-being by actively listening, providing encouragement, and fostering a positive environment. Work collaboratively with family ma=embers, carers and other professionals to provide holistic care and ensure a unified approach to support. Register with Social Care Wales and adhere to its Code of Practice for social care workers. Comply fully with Thrive Group Wales policies, procedures and systems, ensuring best practices in all aspects of work and team compliance.

Essential Requirements



QCF 3 or equivalent and are willing to work towards QCF4/5. Basic numeracy skills along with the ability to communicate effectively both verbally and in writing with colleagues at all levels and on a multi-agency basis. Experience of working in social care. Full UK driver's licence and access to a car Ability and willingness to support people to develop and maintain supportive links and network within their communities. Ability to work collaboratively within a team and work independently, take initiative, and make informed decisions in line with organisational policies and the best interests of individuals. Strong understanding of the challenges faced by vulnerable individuals. Ability to understand Person Centred Plans and how these translate in to service delivery and best practice. Focusing on how individuals like to be supported and what is important for them. A commitment to undertaking cleaning regimes to a high standard. Excellent customer service and communication skills, with the ability to engage effectively with individuals, families, and professionals. Leadership and motivational skills. Communication and interpersonal skills. Works as part of an enthusiastic, highly motivated team with a strong value base, considerate to colleagues and customers, always acting professionally with respect for all. Willingness to engage in continuous learning and professional development, keeping skills and knowledge current.
