This Hepatology Nurse will care for non- cirrhotic and cirrhotic patients. Care is delivered in autonomous nurse led clinics. which includes remote clinics, face-to-face clinics, and MDTs.
Evaluate named liver diseases with the Team to ensure care is provide under BSG/BASL guidelines is 'right time, right place, right person'
Build links with other hospital and community services which see patients with Metabolic dysfunction-associated diseases and Haemochromatosis patients
Cover colleagues if needed - Paracentesis Service care, (remotely and Day Cases Units) : Hepatology/HPB/ Tiertary MDTs : Co-ordinate the HCC Surveillance Service and maintained remotely. It is delivered with the support of primary care, radiology and within our patients department.
Provide support and educational links between primary, secondary and tertiary care. Link between Hepatology outpatient care and Gastro Ward in-patient care.
The role will develop through the requirement of patients, team capabilities and department need, whilst ensuring Trust objectives are met. The Hepatology Nurses Department works and is led by the Gastroenterology Physicians and Lead Hepatology Nurse.
The post holder will collaborate with Hepatology nurses and consultants to ensure NCIC meets Key Performance Indicators defined by BSG/BASL
To work alongside the Hepatology team, and with their support care for patients with liver disease including Haemochromatosis, Metabolic dysfunction-associated steatotic liver disease, viral hepatitis, alcohol related, and compensated cirrhosis. The post holder would lead on two of the liver diseases named above and support on hepatocellular carcinoma surveillance and programme of care for patients with ascites.
To provide nurse led Hepatology clinics across the Trust with support from team via telephone/email.
Be responsible for the care delivery for patients undergoing named liver diseases for non-cirrhotic and stable compensated cirrhosis and to meet the needs of patients.
Manage patients within the BASL/BSG guidelines and care bundles. The post holder will develop protocol/SOPs within the Trust for the named liver diseases.
To establish effective links with Hepatology multi-disciplinary team (MDT) in order to ensure that when liver disease/damage is progressing it is detected, there is timely presentation and transfer of patients to the MDT.
To ensure patient records are completed to include results of all diagnostic investigations, onward referrals, and clinical communication
Work collaboratively & co-operatively with clinical colleagues to develop services and ensure quality of care is delivered.
Work within local guidelines and those of NHS England/Operational Delivery Networks, BASL, and BSG & NICE
